• 200mg/vial

Diffuse Large B-cell Lymphoma (DLBCL)

Indicated in combination with lenalidomide for treatment of relapsed or refractory DLBCL (not otherwise specified), including DLBCL arising from low-grade lymphoma, in adults ineligible for autologous stem cell transplant (ASCT)

Cycle 1: 12 mg/kg IV on Days 1, 4, 8, 15, and 22 of 28-day cycle

Cycles 2 and 3: 12 mg/kg IV on Days 1, 8, 15, and 22 of each 28-day cycle

Cycle 4 and beyond: 12 mg/kg IV on Days 1 and 15 of each 28-day cycle until disease progression or unacceptable toxicity

PLUS

• Lenalidomide 25 mg PO daily on Days 1-21 of each 28-day cycle for maximum of 12 cycles

• Refer to lenalidomide prescribing information for dosage recommendations, including dosing in renal insufficiency and thromboprophylaxis for venous and arterial thrombotic events
